About Discharge Ready Home

Discharge Ready Home helps families prepare the physical home before someone returns from the hospital or rehabilitation. We focus on the everyday parts of the home that can suddenly become more difficult during recovery—entrances, bathrooms, bedrooms, walking areas, lighting, and other frequently used spaces.

Preparing the Home Before They Come Home

Then use a simple bullet list:

  • Entrances, steps, thresholds, and doorways
  • Bathrooms, showers, tubs, and toilets
  • Bedrooms and recovery spaces
  • Walking paths and furniture arrangements
  • Lighting and nighttime visibility
  • Everyday living areas
  • Common physical barriers that may affect mobility or access

The Home Readiness Check

The Home Readiness Check walks families through a series of simple questions about the physical home. Based on those answers, it helps identify areas that may deserve a closer look before someone returns from the hospital or rehabilitation.

The check is educational and practical. It does not require medical records and does not determine whether someone is medically ready for discharge.

Home Preparation, Not Medical Advice

Discharge Ready Home provides general educational information about preparing the physical home. Our guides and Home Readiness Check do not evaluate a person’s medical condition, determine whether someone is ready for discharge, certify that a home is safe, or replace an individualized assessment.

When a recommendation depends on a person’s abilities, mobility, transfer needs, equipment, or medical restrictions, families should confirm the appropriate approach with the healthcare professionals involved in that person’s care.

Who Created Discharge Ready Home

Discharge Ready Home was created to help families recognize practical home-preparation issues before someone returns from a hospital or rehabilitation facility.

The resource focuses on the physical home: entrances, walking routes, bathrooms, bedrooms, lighting, mobility access, everyday living spaces, and other conditions that may affect how easily someone can return home.

Our goal is to turn common home-preparation questions into clear, practical information families can use alongside the guidance they receive from healthcare professionals.

How We Create and Review Our Content

ischarge Ready Home creates educational content around practical home preparation, accessibility, and the return-home environment.

We aim to make each guide clear, useful, and careful about the limits of general information. We distinguish between simple home-preparation steps and situations that may require guidance from an occupational therapist, physical therapist, nurse, physician, discharge professional, contractor, or other qualified professional.

We review our content for clarity, practical usefulness, and appropriate boundaries before publication and update it when better information or guidance becomes available.

Unless a page specifically states otherwise, Discharge Ready Home content has not been individually reviewed or approved by a licensed healthcare professional.
